Income Documentation Guidelines

INCOME TYPE
DEFINITION

ACCEPTABLE DOCUMENTATION

Earned Income

The receipt of wages, salary, commission, or profit from an individual's performance of work or services

REQUIRED ARE THE FOLLOWING:

  • Income Tax Form for most recent tax year
  • In the absence of a federal tax return, an applicant may submit:
    1. Pay stubs, or
    2. W-2, or
    3. Other documents

COMMENTS:

  • Income documentation must be reflective of current income. If document provided does not validate the individual's current income, additional documentation should be requested
  • Proof of income must be provided for each family member listed on the renewal request as having earned income.
  • Other documents must include employee name, company name, pay period, and gross income (before deductions).
  • Written statements from employer must include employee name, company name, employer signature & phone number, as well as pay period, and gross income (before deductions).

ACCEPTABLE DOCUMENTATION

Self Employment Income

Income received by an individual who owns a business or otherwise engages in a private enterprise is considered self-employed

REQUIRED ARE THE FOLLOWING:

  • Income Tax Form for most recent tax year
  • In the absence of a federal tax return, an applicant may submit:
    1. Business Ledger/Receipts, or
    2. W-2 forms, or
    3. Income Tax Form 1099, or
    4. Work Calendar, or
    5. Bank Deposits

COMMENTS

  • Income tax returns must be for most recent tax year.
  • Income of a parent-owned corporation is treated as earned income from self-employment.
  • Documentation must be provided for each family member indicated on the application as having self-employment income.

Unearned Income

Income for which there is no performance of work or services

REQUIRED ARE:

  • Income Tax Form for most recent tax year, if applicable
  • Proof of unearned income (see rows below for types of acceptable proof)

COMMENTS

  • If unearned income is for child only (ex: Child Support), no 1040 or income tax form is required.
  • If unearned income is for Social Security benefits or Social Security Income, no 1040 or income tax form is required.

ACCEPTABLE DOCUMENTATION

Child Support

REQUIRED ARE ONE OF THE FOLLOWING:

  • Child Support court order
  • Copy of check from Non-custodial parent
  • Copy of check from Dept of Revenue (DOR) Child Support Enforcement (CSE) Disbursement Unit
  • Print out from County Clerk of the Court
  • Current written statement from Non-custodial parent

COMMENTS

  • The child support should be indicated as income for the child for which the support was received.
  • Written and signed statement from non-custodial parent or a copy of a check from the non-custodial parent indicating a different amount from the court statement is acceptable to override court statement amount provided it is current. Current means absent parent's signature is dated within 60 days prior to receipt.
  • Documentation must be provided for each family member indicated on the application as receiving child support.

Social Security Benefit (including SSDI and Death Benefits)

REQUIRED ARE:

Social Security Administration Award Letter

COMMENTS

  • If the social security benefit is Social Security Disability Income (SSDI), then the spouse and all children usually receive smaller income checks. This should be included as income on the Renewal Request.
  • Award letter is required because check may take out certain deductions making it a net amount rather than gross.

REQUIRED ARE:

  • Unemployment benefits
  • Workers Compensation
  • Annuity, pension, retirement or disability
  • Veteran's benefits
  • Railroad retirement

REQUIRED ARE THE FOLLOWING:

  • Income Tax Form for most recent tax year, if applicable
  • In the absence of a federal tax return, an applicant may submit:
    1. Awards Letter, or
    2. Check Stub

COMMENTS

  • Document must indicate gross income, not just net.
  • Document must indicate what period of time payment is for and to whom.
  • If more than one kind of "other" income is listed on the Renewal Request, we need a form of documentation on each.
  • Loans are not considered income.

  • Document must indicate what period of time payment is for and to whom.
